What is the MFP Demonstration?

The MFP Demo is a federally funded program that covers a set of services that help people with disabilities and older adults to move from an institution to a qualified community setting.

Certain other MassHealth covered home and community-based services are also available for eligible individuals to support the opportunity to live and work in their communities. People will have choice and help deciding where to live.

What are the MFP Demonstration requirements?

To qualify for the MFP Demo, a person must

  • Be an eligible MassHealth member or meet certain financial criteria to qualify
  • Be living in a qualified nursing facility or long-stay hospital for at least 60 consecutive days, including Medicare rehabilitation days
  • A resident of Massachusetts
  • Be 18 years old or older and be disabled, or be age 65 or older, and
  • Transition to an MFP qualified residence in the community, which may include returning to your home.
