Foreign Insurance Company Name Change Instructions

A foreign insurance company with authority to transact insurance business in Massachusetts that changes its name must provide the following:
  1. A certified copy of Amended Articles of Incorporation reflecting the new company name
  2. A copy of the approval of the name change from the insurer’s domiciliary state insurance department
  3. The effective date of the name change
  4. The non-refundable fee can be paid via OPTins for the following nonrefundable amount:
Licensed Companies without Designation 51 or 54Cost
Amended Company License Fee$ 250.00
Amended Articles of Incorporation Filing Fee$ 65.00
Total Licensed Companies without Designation 51 or 54$ 315.00 
Licensed Companies with Designation 51 or 54Cost
Amended Company License Fee$ 279.00
Amended Articles of Incorporation Filing Fee$ 65.00
Total Licensed Companies with Designation 51 or 54$ 344.00
Surplus Lines Authorized Insurers or Accredited ReinsurersCost
Amended Articles of Incorporation Filing Fee$ 65.00     
  1. A copy of the W9 Form, reflecting the new company name should be sent to DOI Assessments.
