Volunteer Fire Capacity (VFC) Program Application and Forms

Please fill out each form completely and correctly. 

The submission deadline for the 2024 VFC Grant is January 10th, 2025.

Each submission must include the following completed and signed documents:

  • VFC Application
  • VFC Standard Contract
  • A-133 Part B form
  • Contractor Authorized Signatory Listing
  • AD-1048 Certification Regarding Debarment 

Complete the documents and submit them by email to alex.belote@mass.gov, or sending paper copies to:
