Pharmacist License – Reactivation

Reactivate a pharmacist license if it has been expired for more than two years. This does not apply to licenses that have been suspended or revoked by action of, or agreement with the Board.

What you need:

  • Documentation of continuing education (CE) hours for the most recent two full calendar years.
  • Verification from each state of pharmacist licensure (current, expired, and inactive) verifying license status and identifying any disciplinary action since the date of issuance of each license.
  • Payment of all past due license fees and reactivation fee.
  • Following application approval, you will be eligible to register for the Multistate Pharmacy Jurisprudence Examination (MPJE®). See here for review guide.
