For State Owned Buildings
The BBRS Staff and state building inspectors have created building permit application forms for state owned building projects. The Office of Public Safety and Inspections requires an electronic submittal of all documents required by the application, including construction documents, if applicable. To facilitate the electronic submittal process the applicant may use the fill-in form in MS Word format. For those who choose the more traditional approach you may open and print the PDF format, fill it in with legible print, scan the finished document, and then use it for the electronic submittal.
IMPORTANT NOTE ON PERMIT FEE: The Office of Administration and Finance has approved a new fee schedule that is outlined in section 12 of the application form.

For All Other Buildings
The BBRS Staff and municipal building inspectors have created two standard Office of Public Safety and Inspections (OPSI) building permit application forms. Municipalities may add to or use a variant of this form but it must contain at least the same information so that the application process and information requirements are similar across the state. It is suggested that the applicant contact the municipality to which the building permit application will be submitted to determine if the OPSI form is acceptable.
