The Organ Transplant Fund consists of donations made by taxpayers on their State Income Tax Returns or directly to the Fund. Residents of the state are eligible for assistance with expenses not covered by insurance if they have received a functioning organ and have an Adjusted Gross Income of 300% of the Federal Poverty Level or below.
The Organ Transplant Fund may be used for any medical, rehabilitation or other costs deemed necessary to maintaining the transplant recipient health and allograft function. Use of the funds for recipients include, but are not limited to, reimbursement of out-of-pocket costs or partial costs of: transplant related drugs, health insurance premiums, medical co-pays, and transportation to medical and post-operative appointments. The Fund shall not pay for items covered by government programs (such as ESRD Program or Medicare) or by your public or private insurance.
The Fund seeks appropriate applicants through indirect publicity and primarily through transplant social workers at transplant hospitals. Applications can be obtained through the social worker where transplants are performed or downloaded here. The application is used to determine medical and financial eligibility.
Applications must include the following:
- The completed application form
- A signed letter from the established transplant center, or current physician overseeing direct care related to the transplant. The letter should include:
- Patient’s diagnosis
- Patient’s status
- Patient’s current level of activity
- A copy of most recent Massachusetts and Federal Income Tax Returns and Schedule HC (health insurance verification form)
The fund is a great help to residents of the Commonwealth. If you are interested in contributing to the fund, there are several ways to do so. First, you may make a contribution on the Massachusetts Income Tax Return forms when you file each year. The amount you contribute may be part of a refund or in addition to the taxes you are paying. Second, you may contribute with a check made payable to the Massachusetts Organ Transplant Fund and sent to the contact information on this page.
