CARES for Kids Program Services

CARES services support eligible members under 21 years of age and their families in care planning and coordination of services across the health, educational, state agency, and social service systems.

MassHealth’s Coordinating Aligned, Relationship-centered, Enhanced Support (CARES) for Kids is a targeted case management (TCM) service that provides a single source of prompt, family-centered care coordination for children and youth with complex medical and social needs.

CARES Program Services

  • Assessment of medical and social needs
  • Development of an individualized care plan
  • Care coordination and family support activities, including referral to resources and services
  • Follow-up activities to ensure that the member’s care coordination needs are met
