The Details

What you need

To complete your application for the Certified Maintenance Mechanic Exam, you will need the following:

  • You must be at least 18 years old
  • 2”x 2” passport-style photo (Massachusetts Residents may elect to authorize OPSI to access their RMV photo)
  • If you are active duty military, a veteran, or a spouse of an active-duty military service member and applying for fee waiver, you will need a copy of your DD-214, or military ID and orders
  • If you are applying for special accommodation, you will need to fill out an exam accommodation request form, available at OPSI exam page, as well as supporting documentation from your doctor.

If you are applying for an exam waiver:

  • A certificate from the National Association of Amusement Ride Safety Officials (NAARSO) or the Amusement Industry Manufacturers & Suppliers, International (AIMS).

Fees

Application processing fee is non-refundable.

When you are finished with your application, you will be redirected to our payment processor for payments. There is a convenience fee for all online transactions charged by our e-payment vendor. Online credit card transactions incur a 2.39%, debit card transactions incur a 2.09% convenience fee, or ACH/EFT (electronic checks) transactions incur a fee of $0.35.

Next steps

Application approval and exams

Once your application has been approved, OPSI will schedule your exam, and you may not request to be re-scheduled, except for unforeseen illness (doctor’s note will be required), or death of an immediate family member.
