What you need

Before you apply, check our eligibility requirements to see if you are eligible to receive a MassEDP telephone.

Depending on your disability, you will need to send your application to one of our Commissions. 

Send your application to the Massachusetts Equipment Distribution Program if your primary disability is:

  • Deaf
  • Hard of hearing
  • Mobility
  • Vision
  • Speech
  • Cognitive
  • Legally blind
  • Deaf/blind

How to apply

To apply by mail:

  • Print and fill out the application
  • Bring your completed application to your doctor, and have them sign it for certification of your disability
  • Submit your application to the Massachusetts Equipment Distribution Program.

Next steps

After your disability is certified by the Commission

Once the Commission certifies your disability, one of our representatives will contact you. We will set up an appointment for you to receive your MassEDP equipment.
