What you need   for Apply for a MassEDP telephone

Before you apply, check our eligibility requirements to see if you are eligible to receive a MassEDP telephone.

Depending on your disability, you will need to send your application to one of our Commissions. 

Send your application to the Massachusetts Commission for Deaf or Hard of Hearing if your primary disability is:

  • Deaf
  • Hard of hearing
  • Mobility
  • Vision
  • Speech
  • Cognitive

Send your application to the Massachusetts Commission for the Blind if your primary disability is:

  • Legally blind
  • Deaf/blind

How to apply   Apply for a MassEDP telephone

To apply by mail:

  • Print and fill out the application
  • Bring your completed application to your doctor, and have them sign it for certification of your disability
  • Submit your application to the appropriate Commission

If you are sending your application to the Commission for Deaf or Hard of Hearing, mail to:

Next steps   for Apply for a MassEDP telephone

  1. After your disability is certified by the Commission

    Once the Commission certifies your disability, one of our representatives will contact you. We will set up an appointment for you to receive your MassEDP equipment.

  2. Free installation

    You may be eligible for a free installation of your telephone if your primary disability is:

    • Blind

    • Deaf/blind

    • Cognitive

    • Mobility

    If you do not qualify for free installation, you can pick up your equipment at one of our sites or by mail.
