Fees

| Minimum Wage Waiver permitting payment of not less than 80% of the basic minimum wage to minors attending secondary school who work part-time in hospital wards, school and college dining rooms and dormitories where the ratio of one minor to five adult persons working in these areas is maintained. | $100 per year |
| Application for Seasonal Business Determination for Overtime Waiver | $200 per season |
| Approval for Employee Uniform Deposits and Waivers | $100 per year |
| Application for Waiver of Minimum Wage for Student Employees Enrolled In and Employed by a Bona Fide Educational Institution | $100 per year |
