What you need   for Apply for LSP Change of Status

You may discontinue the performance of hazardous waste site cleanup professional services for no longer than two years pursuant to 309 CMR 9.00. Once you have submitted the notice you must or each site for which you are the LSP-of-Record: shall notify MassDEP pursuant to 310 CMR 40.0169, that you are no longer an LSP-of-Record for that site, and you are no longer be eligible to act as, advertise as, or hold yourself out to be a licensed site professional. While on inactive status you must continue to pay the Annual Fee of $225. You may request reinstatement and return to active status by submitting notice of your intent to the Board.
