What you need
The Drug Control Program (DCP) issues the Massachusetts Controlled Substances Registration (MCSR) to community-based program that are licensed, funded, or operated by the Department of Mental Health, Department of Children and Families, adult MassAbility and adult Department of Developmental Services.
You may now apply for or renew your MAP MCSR on the Health Professions Licensing Portal. If this is your first time using this new portal, please visit Health Professions Licensing System User Guide for information on how to get started.
What you need before applying for a new MCSR
Youth sites
A youth program site must submit:
- Copy of site license(s)
- Copy of current Contract with DMH or DCF
A DMH Youth Therapeutic Day Service must submit:
- Copy of current contract
- DMH Letter of support (site address specific)
Adult sites
An adult site must submit:
- Copy of current DDS/DMH/MassAbility license
- And/or current contract with DDS/DMH/MassAbility
- If the license and/or contract do not indicate the site address requesting registration, an additional secondary document indicating DDS/DMH/MassAbility support for the site address is required
