You must notify the Office of Emergency Medical Service (OEMS) of changes to information on your ambulance service license.

What you need   for Change information on an ambulance service license

Upgrade in ambulance service license

Changes in regular operating area or place of business

  • Changes in the regular operating area may include, but is not limited to:
    • Changes in back-up arrangements
  • Changes in the service’s place of business may include, but is not limited to:
    • Change in type of service conducted from that location
    • A new garage location
    • Re-location of company headquarters
  • For these changes, you only need to use Part B and Part D of the application to notify us.

Other important changes you must report

These may or may not require completion of an application form(s).

  • Change in ambulance classification
  • Change in corporate ownership
  • Change in mailing address
  • Change in telephone numbers (emergency and business)
  • Change in service manager
  • Change in affiliation agreement

Substantive changes in information that has been provided on application forms must be reported to OEMS as they occur.

Depending on the change, you may need documentation in writing and/or verbal reporting to OEMS.

If there is any question about the requirements, please call the Inspector for your service.

  1. Complete all parts of the Complete Ambulance Service License Application (All Parts)
  2. Mail completed forms, supporting documents, and fee to:

Changes in regular operating area or place of business

For these changes, you only need to use Part B and Part D of the application to notify us.
