Upgrade in ambulance service or EFR license

If you are upgrading your existing ambulance service license, you must complete all parts of the application. The Office of Emergency Medical Services (OEMS) must receive your complete application and all required fees before your application can be reviewed.

The Office of Emergency Medical Services transitioned to a new online licensing system, the Health Professions Licensing Portal.

You will need to create an account and link your existing license(s) to your new account. If you already have an account, you will still need to link your existing license to your account. If you have not received an email with your “Registration Pin Code”, please contact oems.ambulance@mass.gov. Detailed instructions are below.

Prior to starting the application, please ensure you have the following ready to be uploaded to the application:

  • A Notice of Intent
  • Letter of endorsement from highest elected/appointed municipal official (Municipal Only)
  • A Certificate of Good Standing for the applicant from the Secretary of State’s office from the state in which the corporate headquarters are located (Private Only)
  • A Certificate of Good Standing from the Department of Revenue of the state in which the applicant resides, or the corporate office is based (Private Only)
  • Proof of Accreditation (Critical Care Only)
  • A list of all officers, directors, shareholders and operation managers of the applicant
  • A current copy of the Articles of Organization of the applicant
  • Policies and Procedures
  • EMS Personnel List
  • Affiliation Agreement(s)
  • Weight Verification Slip (additional/replacement vehicles)
    • Not required for Class IV and V ambulances and all EFR vehicles
  • Checklists developed for equipment and supplies
  • 3-year plan detailing how you will achieve and maintain current staffing for licensure

Changes in regular operating area, place of business, additional vehicles, etc.

Changes and additions to service licenses include, but are not limited to:

  • Changes in back-up arrangements
  • Change in type of service conducted from locations
  • A new garage location
  • Re-location of company headquarters
  • A new or replacement vehicle
  • Change in ambulance classification
  • Change in corporate ownership
  • Change in mailing address
  • Change in telephone numbers (emergency and business)
  • Change in service manager
  • Change in affiliation agreement
  • Upgrade in service level

Substantive changes in information that has been provided on applications must be reported to OEMS as they occur.  
Depending on the change, you may need documentation in writing and/or verbal reporting to OEMS.  
If there is any question about the requirements, please call the Inspector for your service.

You may submit your fee using a credit card, debit card, or ACH payment. If you must mail a physical check or money order, please complete and attach payment to the Ambulance Regulation Program Mail Fee Submission Form (PDF) | (DOCX)

Ambulance service licensure - ALS $600 per license
Ambulance service licensure – Critical Care $750 per license
EFR service - EMS first response level only $100 per license
EFR service – BLS $150 per license
EFR service – ALS $200 per license
Ambulance vehicle certification of inspection (with renewal only) $200 each vehicle
EFR-ALS vehicle certification of inspection $50 each vehicle

How to change   Change information or upgrade an ambulance service license

You will need to create an account and link your existing license(s) to your new account. Please note, if you hold licenses/certifications from multiple boards, each license/certification must be linked one at a time.

To create an account, link your certification, and submit your service’s renewal application:

  1. Go to the Health Professions Licensing Portal
  2. Click the “Create Account” button or log in to your existing account
  3. Log in to your account and select “Manage My Licenses and Applications” and then “Link License/Registration to my Account.”
    1. Select your license type from the “License Type” dropdown menu; and
    2. Enter your license number in the “License Number” field.
      1. Please ensure you enter the correct Ambulance Service or EFR License with the newly created pre-fix (“AMB-” for Ambulance Service Licenses, “EFR-” for Emergency First Responder Licenses)
      2. You will be asked to provide your Registration Pin Code
        1. If you did not receive your Registration Pin Code yet, please contact oems.ambulance@mass.gov.  
  4. Once your certification has been linked to your account, select “Show Details”
    1. Upgrade ambulance service license (or EFR):
      1. You will see the option to start a “License Type  – Upgrade Amendment Application
    2. Information changes and additions to service licenses:
      1. Information changes:
        1. You will see the option to start a “License Type - Update General and Service Information
      2. Updates to or additional places of business or vehicles:
        1. You will see the option to start a “License Type - Update or Add Place of Business or Vehicle Amendment
  5. Complete the application, submit payment (if required), then submit. Please note, submission comes after payment.
    1. If you service cannot submit an online payment, please complete and mail the Ambulance Regulation Program Mail Fee Submission Form (PDF) l (DOCX) with your check or money order.
    2. If you submit payment through the mail, once received and entered, you will need to return to your online application to officially submit.
