What you need
To check a license issued to an individual, you need to fill out a form on the License Verification Site. You can search for a license by:
- Profession
- License type
- License number
- License status
- Name
- City
- State
- Zip code
- County
To verify a license issued to a facility, visit the same site and select “Facility Search.” You can search for a license by:
- Profession
- License type
- License number
- License status
- Business name
- Manager name
- City
- State
- Zip code
- County
Note: License information is available from 2001 to present.
