Please follow the instructions on the form and fill it out completely before mailing or faxing it back to our Office of Investigations. Please note that all complaints must be signed. If you wish to file a complaint electronically, please visit the Commonwealth’s ePLACE portal or eLIPSE platform and follow the instructions for creating an account.
How to file
The move to our new licensing system, eLIPSE, has begun for the following boards:
All licensing functions for these boards are now disabled in ePLACE. The new eLIPSE system is expected to be available on Monday, March 30th at 8am.
- For the previously migrated Board of Registration of Real Estate Brokers and Salespersons and the Board of State Examiners of Plumbers and Gas Fitters, licensing functions in the eLIPSE platform will be disabled until Monday, March 30th at 8am.
- For all other users, if you wish to file a complaint electronically, please visit the Commonwealth’s ePLACE Portal and follow the instructions for creating an account.

In cases where it can be proven that a licensee or unlicensed individual or business has violated an applicable statute or regulation or is not safely practicing their trade or profession, DOL may open a complaint and conduct an investigation in coordination with the appropriate board of registration. Grounds for complaints may include, among other things, failure to adhere to standards of practice, fraud, fraudulent procurement of a license, misuse of client funds or records, negligence, practice while impaired by alcohol or drugs, sexual misconduct, and unlicensed practice.
If you are not sure if a licensee’s actions rise to the level of misconduct, you may contact a DOL investigator to discuss your complaint by calling our Office of Investigations at 617-701-8756.
