What you need
Complaints:
- If you want to make a complaint about the care you or someone else received by Emergency Medical Services (EMS) personnel in Massachusetts, or about EMT training, an EMT psychomotor exam or a mobile integrated health (MIH) or Community EMS program you may file it with DPH/OEMS by completing the online Office of Emergency Medical Services Complaint form.

- EMT-Basics
- Advanced EMTs
- Paramedics
- Instructor/Coordinators
- Examiners
- Accredited EMS Training Institutions
- Ambulance Services
- EMS First Response Services (EFR Services)
- MIH Programs
- Community EMS Programs
- Complaints may be filed against the following:
- NOTE: DPH/OEMS has no jurisdiction over billing complaints. For billing related issues, you may contact the Consumer Advocacy and Response Division of the Massachusetts Attorney General's Office.
How to file
To file a complete please complete the online Office of Emergency Medical Services Complaint form.
If you are unable to submit an Office of Emergency Medical Services Complaint form online, you may:
- Download and complete the Alternate Office of Emergency Medical Services Complaint Form
- Fax your completed form to (617) 753-7320.
If you are unable to submit an Office of Emergency Medical Services Complaint form online, you may:
- Download and complete the Alternate Office of Emergency Medical Services Complaint Form
- Email your completed form to OEMS.Complaints@mass.gov.
- Do not email sensitive information such as social security numbers, dates of birth, patient care reports, etc.
If you are unable to submit an Office of Emergency Medical Services Complaint form online, you may:
