What you need
There are a few reasons you may need to submit documents to the Unemployment Services website:
- Responding to a fact finding request
- Filing a wage correction
- Filing an appeal
- Reporting a change in circumstance
If DUA needs more information from you, you'll get an email or letter notifying you. You can also find the request in the Action center when you're logged into the Unemployment Services website. If you miss the deadline to respond, you may lose your benefits or they could be delayed.
How to respond
Fact finding request
If DUA needs more information from you, you will see an entry about this on your dashboard and in the Action Center when you log into the Unemployment Services website. You may also receive a letter in the mail if you've chosen to be contacted by mail.
To respond to this request online, login into your account, select the appropriate entry from your dashboard, and follow the prompts to see what information is needed and upload documents if needed.
File a wage correction
If you believe DUA has miscalculated your wage, you can file a wage correction appeal from the Unemployment Services website.
- Log into the Unemployment Services website.
- From the dashboard, select "File a wage correction."
- Follow the on-screen prompts to provide the necessary information and upload documents.
File a change in circumstances
If you have a change in circumstances that causes you to be unable to work, you need to inform DUA.
- Log into the Unemployment Services website.
- In the Benefit Details area on the dashboard, select "View more benefit details."
- Select "File a change in circumstances."
- Follow the on-screen prompts to provide the necessary information and upload documents.

File a wage correction
If you've received your monetary determination by mail and believe the DUA has miscalculated your wages, you can fill out the wage correction form that comes attached to your determination letter. Attach any necessary proof and mail it to the address listed on the form:
