What you need

The Bureau of Health Professions Licensure has moved to a new e-licensing platform.

The MCSR -Medication Administration Program (MAP) site closure process has changed from a paper-based process to an online application. You can now process your MAP site closure application online. 

A Massachusetts Controlled Substances Registration (MCSR) is not transferable. The MAP Service Provider must complete the MAP Site Closure process if the MAP site closes, the Corporate Service Provider changes ownership, the individuals at the site are all self-administering, or the MAP site no longer supports individuals, who are supported by DMH/DCF/DDS/MassAbility.

How to submit

Please read and follow the steps below. 

To submit a MAP MCSR site closure application:

  1. Start by logging into your MAP online eLicensing account with your username and password.
  2. Once you have logged into your account, from the Home Page
    • Select: Manage My License & Applications
    • “You will see you currently hold or have # MAP Controlled Substance Registration” (s).
  3. Select: The appropriate MAP MCSR site you wish to process the closure application for.
    • Select: Show Details for that MAP site
  4. The selected site MAP MCSR registration will appear, and you will see the option to Start a Site Closure Application
  5. Complete the entire MAP MCSR Site Closure application and submit for processing
