What you need   for MAP site closure

A Massachusetts Controlled Substances Registration (MCSR) is not transferable. The MAP MCSR issued to the site must be returned to the DPH Drug Control Program (DCP), or destroyed; if the MAP site closes, the Corporate Service Provider changes ownership, the individuals at the site are all self-administering, or the MAP site no longer supports individuals, who are supported by DMH/DCF/MRC/DDS/MRC.

When the MAP Site closes or changes ownership, the site should destroy, or return the original MCSR to DCP with a written letter stating the site is closed with the date of closure and a copy of the completed and signed Attestation Statement Regarding the Disposition of Controlled Substances at MAP Sites.
