Required information:
User Details
  • Legal name
  • Supervisor/Manager
  • Domain
  • Location
  • Agency
  • Division
  • User type (FTE, intern, etc.)
  • Contract expiration date (if applicable)
  • Effective start date
  • Title
  • Floor/Cube/Office
  • Is the user a transfer?  If yes, from where?
Phone Services
  • **Does the user need phone services?
  • *Any additional phone requirements
Account Access
  • **Does the user need an email address?
  • Distribution list(s) and/or network share(s) to be added to (if any) OR whose access to mirror (an existing user)
Desktop Hardware
  • Using existing hardware or requesting new hardware?
    • For existing: the item and asset tag number
    • **For new: list new hardware items
Desktop Software
  • *Any additional software the user needs, and why
