Replacement for a damaged green card
- MAIL the ORIGINAL card to MassDOT Metals Control
- We highly recommend that you make a copy of your Green Card prior to submitting it in case it gets lost in the mail
- Please include contact phone number and email address with a self-addressed return envelope
- IF MassDOT metals control determines that you are current, a new Green Card will be issued. The date on the new Green Card will be the same as the last dated signature
- We will contact you if there are any issues
Replacement for a lost green card
- Retest, following the procedures outlined on the MassDOT welder certification webpage for becoming a certified welder.
- Submit copies of the signatures on your Green Card (if you made them)
- As outlined in our "Helpful Hints," submit photos of your green card, front and back, showing continued and current certification status
- IF MassDOT metals control determines that you are current, a new Green Card will be issued. The date on the new Green Card will be the same as the last dated signature
- Submit, on company letterhead, letters from previous and current employers stating the process and time frame in which you have welded for them.
- These letters need to be inclusive of each six-month period from the date of your latest green card issue
- These letters must be addressed to the MassDOT Metals Control Engineer
- These letters must be signed and dated by an employee of the company who is in a supervisory role and can attest that you welded acceptably in that process
