Replace your disability placard

If your disability placard has been lost, stolen, or damaged, you will need to apply for a replacement.
Processing time varies based upon application volume. Please allow at least 30 business days for processing.

The Details

What you need

To request a replacement disability placard, you will need your: 

  • Name
  • Date of birth
  • Social security number
  • Disability Placard Number (if known)

When you request a replacement placard, your original placard is canceled. Continued use of a cancelled placard is against the law and carries fines and loss of license. If you request more than 1 replacement placard over the course of 12 months, or more than 3 during the 5-year period provided by a permanent placard the RMV may:   

  • Review original documentation/applications that resulted in the original placard being issued
  • Request updated medical information about the customer's condition and ability to operate a motor vehicle safely
  • Request confirmation of application and condition from the customer’s healthcare provider
  • Request disability information from customer’s local city/town office
  • Request customer attestation on proper placard use before a replacement is issued, which may be requested as a notarized letter or notarized affidavit. Letters and/or affidavits may also be requested from others residing with the customer or others involved in instances of confirmed or suspected abuse. 

Fees

There is no fee for a replacement disability placard.
