Transfer your disability placard to a disability plate

If you want to transfer your existing disability placard to a plate, you may apply to do so if you currently have a permanent placard.
Processing time varies based upon application volume. Please allow at least 30 business days for processing.

The Details

What you need

You may convert or transfer your existing permanent disability placard to a disability plate. You must be listed as the primary owner of the vehicle. To complete this, submit a signed, written request for the transfer to the RMV's Medical Affairs. You must be the primary owner of the vehicle in order to have a disability plate issued.

Include the following information in your written request:

  • Current disability placard number
  • Name and mailing address 
  • Reason for the transfer

You do not need to complete a disability plate application.

When the RMV processes your written request, instructions for completing the transaction will be sent to you.

You cannot have both a disability plate and a permanent disability placard.

You may be issued a temporary placard for up to 24 months.
