Request a copy of an autopsy report

We keep reports for the autopsies our office performs. If you are authorized, you may be able to receive a copy of a report by following the steps below.
Most reports are completed in 90 days.

The Details

What you need

First, check to see if you are authorized to receive the report you are requesting.

How to request

Your letter should include:

  • The name of the deceased
  • The date of death
  • Your name and address
  • Your relationship to the deceased
  • If you are giving permission to another person to receive a copy of the autopsy report, provide the name and address of that individual
  • Your signature

Mail a letter with your request to the office where the exam was done.
