We may ask you for more information about your unemployment claim. Find out how to respond to questionnaires and submit additional documents to the Department of Unemployment Assistance (DUA).
Return your completed questionnaire by the deadline stated on it.

What you need

If we need more information from you, we’ll send you a questionnaire and a deadline to respond. If you miss the deadline, you may lose your benefits or your benefits could be delayed. We may request more information from you any time there is a question of eligibility with your claim.

How to respond

  1. Go to your UI Online inbox.
  2. Complete the questionnaire by the due date.
  3. While completing the questionnaire, click the Save button frequently to make sure your information stays in our system.
  4. When you are done, click Submit. If you don’t click Submit, we won’t receive your questionnaire.

You can also send us additional documents and information to support your claim. (See Next steps.)

If you need help, an agent can fill out the information with you by phone. To fill your questionnaire out with an agent, call (617) 626-6800 and select option 2.

  • We will mail a time-sensitive questionnaire to the address you provided on your claim
  • Complete the questionnaire
  • Return the questionnaire postmarked by the due date to:

You may send us additional documents and information to support your claim when you mail back your questionnaire.

Send us additional information to support your claim

 By computer (electronically)

  1. Scan and save your document as a pdf.
  2. Go to your UI Online account.
  3. From your UI Online Homepage, go to View and maintain account information.
  4. Click on Monetary and issue summary.
  5. Click the hyperlink that corresponds to the issue you are responding to.
  6. Select Upload.
  7. Click on the Browse button and select your file. Then enter a brief description of the document.
  8. Select the source, add the received date, and click Upload.

By mail (U.S. Postal Service)

  1. Go to your UI Online account.
  2. From your UI Online Homepage, go to View and maintain account information.
  3. Click on Monetary and issue summary.
  4. Click the hyperlink that corresponds to the issue you are responding to.
  5. Check the box to create a scan cover sheet.
  6. Make sure your pop-up blocker is turned off. Click Submit.
  7. Print the cover sheet and mail it along with your document(s) to DUA.

We need to be able to contact you. It is your responsibility to keep your address, phone number and email address up to date. To do this, log in to your UI Online account and update your account information, or schedule a callback for a member of our department to get in touch with you.
