You will need the deceased person's disability placard and copy of the death certificate. If you do not have a copy of the death certificate and/or a copy of the obituary, please mail the placard with a note stating that the placard holder is ‘deceased'.

Mail your deceased relative’s disabled parking placard with a copy of the deceased’s death certificate (or note) to:

Once the RMV receives the placard, the deceased person's record will be updated and placard will be cancelled.
