The Details

What you need

  1. Complete the Online application

  2. A completed signed Certificate of Character by the Chief of Police in the city you reside.

  3. A copy of your Certified Blind Certificate.

     

How to apply

Fee: Complimentary 

We only accept payment by money order or check (personal, business or bank check). Make checks payable to the Commonwealth of Massachusetts.

We do not accept any other form of payment including cash and credit/debit cards.
