What you need   for Learn how to Upload Documents into your UI Online account

If you are looking to upload documentation into your UI Online account, make sure that you have saved the document as a PDF.  UI Online will only accept documents in PDF format.

1. Once you have logged into your UI Online account, click on View and Maintain Account Information.

2. Click on Monetary and Issue Summary.

3. Click on the Issue Identification Number hyperlink that is associated with the Issue Type that you want to upload documentation to.

4. Click on the Upload radio button. Note: You only need to click the check box if you are looking to mail in documentation. When you mail in documentation you must include the coversheet for us to be able to easily identify your claim.

5. Click on Choose File. Remember: You must choose a file in PDF format.

6. Include a brief description of the file and click Submit.

7. UI Online will display a confirmation page if your upload was successful.
