What you need for Submit additional information to support your unemployment claim
To upload or mail additional documents to DUA, you will need to log in to your UI Online account using your Social Security number (SSN) and password.
If you wish to upload your supporting documents online, you will need to scan each document and save the files as a PDF.
How to submit Submit additional information to support your unemployment claim
- Scan and save your document(s) as a pdf.
- Log in to your UI Online account.
- From your UI Online homepage, go to View and maintain account information.
- Click on Monetary and issue summary.
- Click the issue you are responding to.
- Select Upload.
- Click on the Browse button and select the file you wish to upload. Enter a brief description of the document.
- Select the source, add the received date, and click Upload.
- Log in to your UI Online account.
- From your UI Online homepage, go to View and maintain account information.
- Click on Monetary and issue summary.
- Select the issue you are responding to.
- Check the box to create a scan cover sheet.
- Make sure your pop-up blocker is turned off. Click Submit.
- Print the cover sheet and mail it along with your document(s) to DUA.
