UIC Class V Well: Completion of Construction Notification Form

Use this form to notify MassDEP that your UIC Class V well is complete and ready for use. You cannot begin discharging to the well until MassDEP approves its use.

What you need   for UIC Class V Well: Completion of Construction Notification Form

You must submit this notification form if you have constructed a UIC Class V well in order to obtain approval to begin discharging to the well.

If you originally filed the registration of your well on-line via eDEP, you will need to use eDEP to complete this notification form. If you originally filed the registration of your well on paper, you will need to print out the notification form and mail it to the MassDEP UIC Program.

  1. Review the Instructions

    The first set of instructions is for completing the application by paper.

    The second set of instructions is for completing the application electronically through eDEP.

    If you cannot use eDEP to file your well-completion notification, you may fill out and save the Microsoft Word form on your computer. The PDF version has to be printed out and completed by hand.

  3. Submit your Notification Form
