What you need   for UIC Class V Well Post-Closure Notification Form

You must submit this form if you have completed the closure of a UIC Class V well, in order to obtain final approval of the closure activities from MassDEP.

NOTE: If your original BRP WS 06 UIC registration application was filed through eDEP then you should not be using this downloadable form. Instead, complete and file the same form that is available through your eDEP account.
