Dental Assistant License Reactivation
- Payment of $102 or $147 ($45 renewal fee for one or two two-year licensure cycle(s) plus a one-time-only $57 late fee);
- A color digital photograph (no smaller than a standard 2x2 passport photo);
Proof you completed 12 or 24 CEUs during the preceding one or two licensure cycle(s) depending upon when your license expired including proof you completed a course on the CDC Guidelines for Infection Control in the Dental Healthcare Setting and proof you have are currently certified in Basic Life Support for Healthcare Providers;
NOTE: Licensees may only complete up to 1/2 of the required CEUs through self-study online courses. The remaining 1/2 must be completed by attending live, in person courses OR by completing live, interactive online webinars. - Your answer sheet from the dental assistant ethics and jurisprudence exam;
- A current copy of your resume/practice history; and
- A completed, signed and notarized CORI form.
Dental Hygienist License Reactivation
- Payment of $117 or $177 ($60 renewal fee for one or two two-year licensure cycle plus a one-time only late fee of $57);
- A color digital photograph (no smaller than a standard 2x2 passport photo);
Proof you have completed 20 or 40 CEUs during the preceding one or two licensure cycles (20 CEUs for each cycle) depending upon when your license expired including mandatory coursework as follows:
- A course, either online or in person, on the CDC Guidelines for Infection Control in the Dental Healthcare Setting; and
- Proof of your current certification in Basic Life Support for Healthcare Providers
NOTE: Licensees may only complete up to 1/2 of their required CEUs through self-study online courses. - A current self-query report from the National Practitioner Data Bank;
- Your answer sheet from the ethics and jurisprudence exam;
- A current copy of your resume or curriculum vitae;
- A completed, signed and notarized CORI form; and
- Official, original letters of good standing from any state licensing agency, including other Massachusetts licensing agencies, and other state licensing agencies if you have ever been issued a license elsewhere in Massachusetts or the US. Please note this requirement is not limited to dental licenses but includes all licenses such as teaching licenses, real estate licenses, etc.
Dentist License Reactivation
- Payment of $417 or $777 ($360 renewal fee for one or two two-year licensure cycles plus a one-time only late fee of $57);
- A color digital photograph (no smaller than a standard 2x2 passport photo);
Proof you have completed 40 or 80 CEUs during the preceding one or two licensure cycles (40 CEUs for each cycle) depending upon when your license expired including mandatory coursework as follows:
- A Board-approved course on safe and effective opioid prescribing/pain mgmt;
- A course, either online or in person, on the CDC Guidelines for Infection Control in the Dental Healthcare Setting; and
- Proof of your current certification in Basic Life Support for Healthcare Providers.
