Board of Registration in Dentistry License Reactivation Guide

If your license has expired and you’ve exceeded the 90-day grace period, you are required to reactivate your license.

Please visit our e-licensing system at:- https://healthprofessionlicensing.mass.gov/login-register in order to link your expired license to your account.  If you do not have an account, you can create one. If you cannot recall your username or password, please call the IT Help Desk at 617-973-0935.  Please do not create more than one account – this may cause delays in processing.

Select your license type from the list below.  Once you have all the required documents from that list prepared for upload, you will be able to fully submit your “reactivation” application on the licensing portal.  Do not forget to complete your payment and submit, or your application will not be reviewed.

Dental Assistant License Reactivation

1. Payment of $102 or $147 ($45 renewal fee for one or two two-year licensure cycle(s) plus a one-time-only $57 late fee);
2. A color digital photograph (no smaller than a standard 2x2 passport photo);

3. Proof you completed 12 or 24 CEUs during the preceding one or two licensure cycle(s) depending upon when your license expired including proof you completed a course on the CDC Guidelines for Infection Control in the Dental Healthcare Setting and proof you have are currently certified in Basic Life Support for Healthcare Providers;

   NOTE: Licensees may only complete up to 1/2 of the required CEUs through self-study online courses.  The remaining 1/2 must be completed by attending live, in person courses OR by completing live, interactive online webinars.  Information on CEUs can be found here: https://www.mass.gov/info-details/continuing-education-for-dentistry.

4. Your answer sheet from the dental assistant ethics and jurisprudence exam;
5. A current copy of your resume/practice history; and
6. A completed, signed and notarized CORI form.

Dental Hygienist License Reactivation

1. Payment of $117 or $177 ($60 renewal fee for one or two two-year licensure cycle plus a one-time only late fee of $57);
2. A color digital photograph (no smaller than a standard 2x2 passport photo);

3. Proof you have completed 20 or 40 CEUs during the preceding one or two licensure cycles (20 CEUs for each cycle) depending upon when your license expired including mandatory coursework as follows:

   1. A course, either online or in person, on the CDC Guidelines for Infection Control in the Dental Healthcare Setting; and
   2. Proof of your current certification in Basic Life Support for Healthcare Providers

   NOTE:  Licensees may only complete up to 1/2 of their required CEUs through self-study online courses.  The remaining 1/2 must be completed by attending live, in person courses OR by completing live, interactive online webinars.  Information on CEUs can be found here: https://www.mass.gov/info-details/continuing-education-for-dentistry.

4. A current self-query report from the National Practitioner Data Bank;
5. Your answer sheet from the ethics and jurisprudence exam;
6. A current copy of your resume or curriculum vitae;
7. A completed, signed and notarized CORI form; and
8. Official, original letters of good standing from any state licensing agency, including other Massachusetts licensing agencies, and other state licensing agencies if you have ever been issued a license elsewhere in Massachusetts or the US.  Please note this requirement is not limited to dental licenses but includes all licenses such as teaching licenses, real estate licenses, etc.

Dentist License Reactivation

1. Payment of $417 or $777 ($360 renewal fee for one or two two-year licensure cycles plus a one-time only late fee of $57);
2. A color digital photograph (no smaller than a standard 2x2 passport photo);

3. Proof you have completed 40 or 80 CEUs during the preceding one or two licensure cycles (40 CEUs for each cycle) depending upon when your license expired including mandatory coursework as follows:

   1. A Board-approved course on safe and effective opioid prescribing/pain mgmt;
   2. A course, either online or in person, on the CDC Guidelines for Infection Control in the Dental Healthcare Setting; and
   3. Proof of your current certification in Basic Life Support for Healthcare Providers.

   NOTE:  Licensees may only complete up to 1/2 of their required CEUs through self-study online courses.  The remaining 1/2 must be completed by attending live, in person courses OR by completing live, interactive online webinars.  Information on CEUs can be found here: https://www.mass.gov/info-details/continuing-education-for-dentistry.

4. A current self-query report from the National Practitioner Data Bank;
5. Proof of your enrollment with MassHealth as either an approved billing provider OR as an ORP (Order, Refer, Prescribe) non-billing provider;
6. Your answer sheet from the ethics and jurisprudence exam;
7. A current copy of your resume or curriculum vitae;
8. A completed, signed and notarized CORI form; and
9. Official, original letters of good standing from any state licensing agency, including other Massachusetts licensing agencies, and other state licensing agencies if you have ever been issued a license elsewhere in Massachusetts or the US.  Please note this requirement is not limited to dental licenses but includes all licenses such as teaching licenses, real estate licenses, etc.
