CHEI has partnered with three regional Data & Action Support Providers across Massachusetts to offer capacity building and assistance in using the Community Health Equity Survey (CHES) data for action. The purpose of the Data & Action Support model is to share data back with community-based organizations, municipalities, health systems, and other partners. Providers will offer assistance in using data to inform programming, apply for grant funding, and advocate for policies that reduce health inequities. Data & Action Support Providers are available to:
- Fulfill customized CHES data requests for your geography and/or community of focus
- Support you in engaging residents from your community to contextualize findings and prioritize action steps
- Provide capacity building and resources to take action to address health inequities
