Learn about provider application fees

When the provider types listed below enroll, reenroll, or add a new service location in MassHealth, they may have to pay an application fee.

Background

Federal law requires that the provider types listed below that are enrolling, reenrolling, or adding a new service location into a Medicaid program pay a federal application fee.

Who must pay the fee?

The following provider types must pay the provider application fee (unless paid to Medicare or another state Medicaid program) when enrolling, re-enrolling, or adding a new service location. For the calendar year 2025, the application fee is $730.  

  • Acute inpatient hospitals
  • Chronic inpatient hospitals
  • ICF-MR state schools (intermediate care facility for individuals with intellectual disabilities)
  • Psychiatric inpatient hospitals
  • Semi-acute inpatient hospitals
  • Skilled nursing facilities that do not participate in Medicare

All other provider types are exempt.

Important information about the fee

To pay the fee, you must use the secure payment site.

Please be sure to print a copy of your payment confirmation. You must submit the payment confirmation with your application.

MassHealth does not accept paper checks for the application fee.

The application fee is non-refundable. If you don't meet the participatory requirements, or don't submit the required documentation within the stated time frame, resulting in the application being denied, then the fee will not be refunded. Any further applications for enrollment or reenrollment will require a new application fee.

After Paying the Fee

Please print a copy of your confirmation of payment and submit it with your application.
