What you need for Apply to become a MassHealth provider

Know your provider enrollment choices 

Only MassHealth providers are reimbursed for services provided to MassHealth members. Therefore, providers must be approved as a participating provider before claims can be submitted for payment.

Your application process may be different depending on the type of program you choose. See which application applies to you. 

MassHealth Provider Enrollment 

Review the applicable MassHealth regulations before requesting an application, so you understand the program requirements.

For enrollment or more information, go to the Provider Online Service Center or call the MassHealth Customer Service Center at (800) 841-2900. 

You may need to pay an application fee. Review our fee guidelines

Dental Programs

Dental programs include dentist, dental clinic, dental schools, among others. Go to the MassHealth dental program for enrollment and a complete list of dental programs.   

Long-term Services and Supports (LTSS) Programs

LTSS programs include skilled nursing facility, hospice, home health, durable medical equipment, and adult day health, among others. Go to the MassHealth Long-term Services and Supports (LTSS) Provider Portal for enrollment and a complete list of LTSS programs.   

Ordering, Referring, and Prescribing (ORP)

Ordering, referring, and prescribing providers are providers who order, refer, or prescribe services for MassHealth members, but are not eligible or do not wish to enroll as a fully participating providers and bill MassHealth for services rendered to MassHealth members.

Go to the ACA Section 6401—Ordering, Referring, and Prescribing Provider Requirements page for a complete list of provider types authorized to order, refer, or prescribe, and for enrollment instructions.

Qualified Medicare Beneficiary (QMB) Only

QMB-only providers are not otherwise eligible for or do not want to enroll as a full MassHealth provider. Only crossover claims for members with both Medicare and MassHealth coverage can be submitted.

  • Community health center and transportation providers cannot apply online

  2. Fill out the application.
  3. Mail it to:
