Name change is the legal process of changing your name. By going through a court name change proceeding.

You're required to file this form whenever a petition or complaint is filed or issued in a case involving the care, custody, visitation or name change of a child. Assent to Petition to Change Name of Minor (CJP 30) 

Revised 5/6/24 - Use this form when a minor child's legal parent or court-appointed guardian wants to assent to the petition using a separate form.

Motion (CJD 400) 

If you have a reason the notice shouldn't be published, file a Motion to waive publication. An affidavit (a sworn statement) must be filed with the motion explaining why you don’t want the notice published. You may have to go before a judge to present your reasons.
