Here you will find a listing of Underground Storage Tank (UST) forms and applications administered by the Massachusetts Department of Revenue (DOR).

Effective January 1, 2018 applications for reimbursement, eligibility, reconsiderations, and conferences must be filed electronically through eUST.  For more information about eUST, please visit our Electronic Filing System – eUST page.

Also effective January 1, 2018 all Delivery Fee returns (Form 500) must be filed electronically through MassTaxConnect. Applications for Delivery Fee abatements for returns filed prior to January 1 must be filed using the form below. Delivery Fee abatements for returns filed after January 1 can also be submitted by completing an amended return via MassTaxConnect.
