Attention all licensure applicants
Please be advised the Board strongly recommends each licensure applicant mail their applications and supporting documents to the Board. Please do NOT drop off your application in person as we are also short-staffed and are working as quickly as possible to process your application/issue your license. Each time someone on my staff is called to greet a walk-in, it takes valuable time away from processing an application. Please note we do NOT accept applications by fax or by email.
Make sure to include your current email address and cellphone number with your application. My staff will contact you directly via email or by phone if there are any questions about your application.
Please be aware that we moved our entire office to DPH headquarters in downtown Boston in early December. If you are using an older version of an application, it may list our previous address. ALL applications and correspondence with the Board should be mailed to:
