Skip this video New Shared Living Assessment Training Video.We are pleased to share the new Shared Living Assessment April 2025, the Shared Living Assessment Guidebook 2025 and a training video link to understand how the assessment works.
This new shared living assessment replaces all other shared living assessments and is the assessment that is required for all shared living provider agencies to use for individuals new to shared living as well as any individuals in shared living with a significant change in need. We are confident that this new assessment will accurately capture the support needs of all individuals being assessed for shared living. This new 2025 shared living assessment is for all individuals with ID/D and for all ABI-MFP Residential Waiver eligible individuals who are being assessed for shared living.
