Refunds: When a constituent accidently pays twice (or more) for a license, registration or renewals (usually by clicking on the payment tab twice or more), or pays for the wrong type of license, registration or renewal the constituent will be provided a 180-day window from the date of the transaction in which to contact DLS for a refund. Once the 180-day window expires the payment can no longer be refunded.

Minimum Wage Program Payment must be made immediately following application submittal
Application for Waiver of Minimum Wage for Employees with Disabilities $100 per year

Minimum Wage Waiver permitting payment of not less than 80% of the basic minimum wage to minors attending secondary school who work part-time in hospital wards, school and college dining rooms and dormitories where the ratio of one minor to five adult persons working in these areas is maintained.

Division of Apprentice Standards Payment must be made immediately following application submittal
Sponsor Fee $300 per employer/year
Apprentice ID card $35 per apprentice/per year
Dispensing Optician Sponsor and Apprentice Application Package $40 per apprentice (one‐time fee)
Sponsor verification $50 per employer/per request

Employment, Placement, and Staffing Agencies Program
Payment must be made immediately following application submittal
Licensed Employment Agency (one to four counselors) $300 per year
Licensed Employment Agency (five or more counselors) $550 per year
Registered Placement Agency (main office) $300 per year
Registered Placement Agency (each branch office) $180 per year
