F.O.R. Families (Follow-Up Outreach Referral)

A home visiting program for families transitioning from homelessness to stable housing.

Families receiving Emergency Assistance (EA) from the Division of Housing Stabilization at the Executive Office of Housing and Livable Communities (EOHLC), who are homeless and living in a shelter, are eligible for no-cost home visiting services.

F.O.R. Families Home Visitors are human service professionals with extensive public health expertise, child welfare experience and strong community connections. Addressing the barriers families face in acquiring and maintaining stable housing is the primary goal for the F.O.R Families home visitor. The Home Visitors conduct family assessments and make referrals for additional services relying on their expertise, experience and connections.

F.O.R. Families Home Visitors coordinate services with an array of community-based programs and services such as WIC, Early Intervention, primary health care, domestic violence and substance abuse treatment as well as Housing Assistance workers, the Department of Transitional Assistance, the Department of Children and Families, and the Department of Mental Health when appropriate. They provide information about school enrollment options, food, clothing and transportation resources.

F.O.R. Families is a joint program of the Executive Office of Housing and Livable Communities (EOHLC), and the Bureau of Family Health and Nutrition in the Massachusetts Department of Public Health.
