Application Fee. A fee of $100 is required to process this application. Complete this application first, then pay the fee. A link to make an electronic payment is at the end of this application, or you may pay by check. Checks are payable to the Commonwealth of Massachusetts. Mail the check to the Transportation Oversight Division, DPU, One South Station, Boston, MA 02110.
Balance Sheet. Applicants must submit a balance sheet for the business entity. A sample balance is available on the DPU’s website.
