What you need for Apply for an ambulance service license
If you are applying for an ambulance service license for the first time, you must complete all parts of the application. The Office of Emergency Medical Services (OEMS) must receive your complete application and all required fees before your application can be reviewed.
To apply for a new an ambulance service license, you will need to create an account on the Health Professions Licensing Portal.
Prior to starting the application, please ensure you have the following ready to be uploaded to the application:
- A Notice of Intent
- Letter of endorsement from highest elected/appointed municipal official (Municipal Only)
- A Certificate of Good Standing for the applicant from the Secretary of State’s office from the state in which the corporate headquarters are located (Private Only)
- A Certificate of Good Standing from the Department of Revenue of the state in which the applicant resides or the corporate office is based (Private Only)
- Proof of Accreditation (Critical Care Only)
- A list of all officers, directors, shareholders and operation managers of the applicant
- A current copy of the Articles of Organization of the applicant
- Policies and Procedures
- EMS Personnel List
- Affiliation Agreement(s)
- Any additional agreements such as Memorandum of Understanding, Medication Exchanges, etc.

- Checklists developed for equipment and supplies
- Weight Verification Slip (for vehicles)
- Not required for Class IV and V ambulances and all EFR vehicles
- 3-year plan detailing how you will achieve and maintain current staffing for licensure

How to apply Apply for an ambulance service license
You will need to create an account and link your existing license(s) to your new account. Please note, if you hold licenses/certifications from multiple boards, each license/certification must be linked one at a time. If you are upgrading an existing service, please log in, link, and select “Manage My Licenses and Applications” and “Show Details” on your existing license to begin the upgrade application. Please visit Change information or upgrade an ambulance service license.
To create an account, link your certification, and submit your service’s renewal application:
- Go to the Health Professions Licensing Portal
- Click the “Create Account” button or log in to your existing account
- Log in to your account and select “Apply for a New License” and then select “View More” under “The Office of Emergency Medical Services.”
- Select your license type:
- Ambulance Service License
- Select your license type:
- Complete application, submit payment, then submit. Please note, submission comes after payment.
- If your service cannot submit an online payment, please complete and mail the Ambulance Regulation Program Mail Fee Submission Form (PDF) l (DOCX) with your check or money order.
- If you submit payment through the mail, once received and entered, you will need to return to your online application to officially submit.
