What you need for Apply for LSP Application
Applicants for licensure must provide sufficient information to enable the Board to determine that you meet the qualifications in 309 CMR 3.02. Accordingly, you must demonstrate that you meet minimum education requirements; that you have hazardous waste site cleanup decision making experience; and possesses good moral character.
- LSP Forms 1-3
- Documentation of Educational Degrees
- Four (4) Professional References

The application fee of $245 payment will be requested upon completion of the online application. You will have the option to pay online by credit card or automated clearing house (ACH) or pay by mail. If you select pay-by-mail option, instructions regarding payment process will be emailed when you submit your online application.

Once your application is submitted, the Professional References you identified during the online application process will receive instructions.
If you have been identified as the Professional Reference, please follow the steps below to submit the Professional Reference Form to the LSP Board.

- You will need to upload the completed Professional Reference Form using the link provided. For step-by-step guidance, please see the EEA ePLACE Instructions below.
