Please make sure you list all accounts, systems, networks and applications you would like the user to be removed from.


This item is NOT used to fully deprovision a user. See below for guidance.


Full-time Commonwealth Employees (FTEs): 

  • Use the “HR Deprovisioning” Catalog item to:
    • Fully deprovision a user
  • Use this Catalog item to:
    • Temporarily disable Active Directory access
    • Disable access to other accounts, systems, networks and applications

 

Contractors/Vendors/Guests:

  • Use the “Active Directory” Catalog item to:
    • Fully deprovision a user
  • Use this Catalog item to:
    • Temporarily disable Active Directory access
    • Disable access to other accounts, systems, networks and applications

How to request   Disable Access

Please note: Internal agency IT Liaison (ITL) approval is required in order for EOTSS to process this request.
