IF YOU ARE A TSS SUPPORTED AGENCY please complete this form so we may facilitate the disposal of any IT equipment and/or peripherals including desktops, laptops, tablets, monitors, docks, printers, switches, servers, keyboards, mice, cables, etc.

IF YOU ARE NOT A TSS SUPPORTED AGENCY do not complete this form, instead contact the EOTSS IT Hardware Asset Management Team at TSS-Asset-Disposal-Request@mass.gov and we will put you in touch with our preferred disposal vendor.

The disposal preparation process:  

  • Please be certain all network equipment (servers, switches, etc.) has gone through the proper decommission process (including change management) before requesting disposal.
  • Hard drives can be left in computers and will be removed and destroyed/shredded by our vendor.
  • Computers, laptops, printers, servers, switches, and other larger items should be neatly stacked about waist high directly on pallets/skids when available.
  • Monitor displays should be removed from their stands and, when possible, stacked neatly and upright (like books on a shelf) inside their own Gaylord boxes and palletized.
  • Monitor stands should be separated from the displays and, when possible, put inside their own Gaylord boxes and palletized.
  • Keyboards and mice can be put together, inside their own Gaylord boxes (mail totes or smaller boxes can be used for smaller quantities), and palletized when possible.
  • Cables can be put into their own separate Gaylord boxes (mail totes or smaller boxes can be used for smaller quantities) and palletized when possible.
  • Loose hard drives should be separated and, when possible, put into their own Gaylord boxes (mail totes or smaller boxes can be used for smaller quantities) and palletized.
  • For prep of larger disposals, our vendor can provide and drop off pallets and Gaylord boxes at the disposal location as needed.  For smaller disposals, Gaylord boxes and pallets are not required.

Important disposal information:  

  • There is no charge for the destruction of hard drives if the associated devices are also being disposed of.  If large numbers of loose hard drives (not associated with a device) are being disposed of, they are subject to a charge.
  • Certificates of destruction (COD) can be provided upon request.  CODs referencing hard drives in bulk (weight/quantity) are free of charge.  However, if the hard drives need to be serialized on the COD, a charge will likely be assessed.
  • Smaller equipment disposals will be picked up by the TSS Deskside Team. Larger disposals will be picked up by our disposal vendor.
  • The EOTSS IT Hardware Asset Management Team will handle the disposition of the assets in the ServiceNow Asset Management system

In order to complete this form, you must download, fill out, and attach an OSD Format Approved Spreadsheet listing ALL disposal equipment. Peripherals (mice, keyboards, cables, etc.) can be detailed in the field at the bottom of this form.

Service Level Expectation (SLE)

SLEResponsibilities/Dependencies
Fulfillment: This service was recently introduced as a formal offering in our Product & Service Catalog.  All new request items will be evaluated by EOTSS, who will set and communicate a standard SLE.  In the interim, the response time for this request will be monitored and reviewed based on the complexity of each individual case. End Users can expect to be informed of the estimated response time (SLE) during the intake stage of the request.  

Customer

  • Responsible for adhering to the EOTSS Standard Rules of Engagement.
  • Responsible for submitting accurate information to EOTSS during the intake and discovery process.
  • Sharing complete requirements will ensure that accurate SLEs are provided and met.

EOTSS

  • Responsible for coordinating discovery meeting(s) with the customer.
  • Once all requirements are collected/finalized following intake and discovery, EOTSS will determine the appropriate SLE for the request and communicate to the customer.
