Download, fill out, and fax the discrimination complaint form to MassDOT’s Office of Diversity and Civil Rights at 617-222-3263.

You can either download and print the form or request the form in hard copy at the ODCR office. Fill out and deliver the discrimination complaint form to MassDOT’s Office of Diversity and Civil Rights in-person to:
