Your PIN can be found on documents that DOR sends to you.

If you lost or can't find your PIN, you can retrieve your number. You'll need the following:

  • Social Security number
  • First, middle, and last names
  • Date of birth
  • Mailing address
  • Dependent's (child’s) information:
    • Full name
    • Date of birth.
