What you need   for Renew an ambulance service license

The Office of Emergency Medical Services transitioned to a new online licensing system, the Health Professions Licensing Portal.

You will need to create an account and link your existing license(s) to your new account. If you already have an account, you will still need to link your existing license to your account. If you have not received an email with your “Registration Pin Code”, please contact oems.ambulance@mass.gov. Detailed instructions are below.

If you are renewing an ambulance service license, you must complete all parts of the application. The Office of Emergency Medical Services (OEMS) must receive your complete application and all required fees before your application can be reviewed.

Prior to starting the application, please ensure you have the following ready to be uploaded to the application:

  • Affiliation Agreement(s)
  • Proof of Insurance (Private only)
  • Mutual Aid Agreement(s)
  • EMS Personnel List
  • Weight Verification Slip (additional/replacement vehicles)
    • Not required for Class IV and V ambulances and all EFR vehicles
  • Policies and Procedures
  • Proof of Accreditation (Critical Care only) 

Fees   for Renew an ambulance service license

You may submit your fee using a credit card, debit card, or ACH payment. If you must mail a physical check or money order, please complete and attach payment to the Ambulance Regulation Program Mail Fee Submission Form (PDF) l (DOCX)

Ambulance service licensure - ALS $600 per license
Ambulance service licensure – Critical Care $750 per license
EFR service - EMS first response level only $100 per license
EFR service – BLS $150 per license
EFR service – ALS $200 per license
Ambulance vehicle certification of inspection $200 each vehicle
EFR-ALS vehicle certification of inspection $50 each vehicle

How to renew   Renew an ambulance service license

You will need to create an account and link your existing license(s) to your new account. Please note, if you hold licenses/certifications from multiple boards, each license/certification must be linked one at a time. If you are upgrading an existing service, please log in, link, and select “Manage My Licenses and Applications” and “Show Details” on your existing license to begin the upgrade application. Please visit Change information or upgrade an ambulance service license website.

To create an account, link your certification, and submit your service’s renewal application:

  1. Go to the Health Professions Licensing Portal
  2. Click the “Create Account” button or log in to your existing account
  3. Log in to your account and select “Manage My Licenses and Applications” and then “Link License/Registration to my Account.”
  4. Select your license type from the “License Type” dropdown menu; and
  5. Enter your license number in the “License Number” field.
  6. Please ensure you enter the correct Ambulance Service or EFR License with the newly created pre-fix (“AMB-” for Ambulance Service Licenses, “EFR-” for Emergency First Responder Licenses)
  7. You will be asked to provide your Registration Pin Code
  8. If you did not receive your Registration Pin Code yet, please contact oems.ambulance@mass.gov
  9. Once your certification has been linked to your account, select “Show Details”
  10. You will see the option to start a “License Type  – Renewal Application”
  11. Complete application, submit payment, then submit. Please note, submission comes after payment.
  12. If you service cannot submit an online payment, please complete and mail the Ambulance Regulation Program Mail Fee Submission Form (PDF) l (DOCX) with your check or money order.
  13. If you submit payment through the mail, once received and entered, you will need to return to your online application to officially submit.

If DPH is unable to act upon a properly submitted relicensure application within 90 days, the service may continue to operate until DPH does take action. It's important to note that if a service fails to submit a complete and acceptable application for renewal in a timely manner, the service may NOT continue to operate past the expiration date without written permission by DPH.

If you are approaching your expiration and believe you may lapse, you may request to maintain your current status for the continuance of billing. Please send your request to maintain current status to oems.ambulance@mass.gov. If granted, you shall not expire until the Department takes action.
