What you need for Submit evidence to the Crime Lab
All evidence being submitted to the Crime Lab must be submitted by an authorized agency. The evidence must be identified, inventoried and sealed prior to submission. Packaging material should protect the evidence from contamination, alteration and tampering. Please refer to the guides included on the Evidence Submission page for information regarding standard procedures for packaging evidence to be submitted to the Crime Lab.
You will need:
- An appointment to submit evidence
- Your sealed evidence
- A completed Evidence Submission Form (ESF) or SP295 Drug Submission Form
- A police report (For non-drug submissions only)
