What you need   for WM 05, 06, 07: Surface Water Discharge (NPDES) Individual Permit Application

Under the Surface Water Discharge Permit Program (314 CMR 3.00) and the Massachusetts Surface Water Quality Standards (314 CMR 4.00), these permits protect the public health, welfare, and the environment through the control of industrial and non-industrial wastewater discharges to surface waters of the Commonwealth.

For a plan modification not associated with permit renewal, submit MassDEP Form WM 16.

You must apply for this permit if your facility meets the definitions provided in 314 CMR 3.00, except for those activities defined as exempt in 314 CMR 3.05.

To apply for this permit, you must submit the permit application through ePlace (see below). All required US EPA forms must be submitted to both US EPA and MassDEP.

  1. Review the Instructions

  2. Complete the Application Form (and appropriate attachments)

    Please fill out and save either the Microsoft Word or Acrobat Adobe PDF form on your computer. 

    Important Note: Even when applying online, you will still need to complete a conventional Application Form and upload it as an attachment during the online filing process. Please do not mail in these forms.

  3. Claim your Authorization

    1. If you have an existing NPDES permit, you will first need to do one important step: Claim your Authorization. This process will link your permit to your application and will verify your identity.

      You may fill out and save the Microsoft Word or Adobe Acrobat PDF "NPDES Claim Authorization Form" on your computer. This will also be uploaded as an attachment during the Claim process. 
    2. Claim Online via the EEA ePlace Portal.
      Note: if you are a first-time user, you will need to begin by creating an account.

      See the ePlace account setup instructions and Comprehensive User Manual below. Once MassDEP has approved your claim, proceed to step 4. 
  4. Submit your Application and Fee Payment

    1. File online via the EEA ePlace Portal.
    2. You will have the option to pay online by credit card or automated clearing house (ACH) or pay by mail. If you select pay-by-mail option, instructions on how to do so will be provided when you submit your online application.
